Electronic Medical Records System Information
We are using Electronic Medical Records system to ensure the most efficient care available to our patients.
How do I get a copy of my child's vaccine/shot record?
All requests for vaccine/shot records must be submitted to our office in writing. You may fax us at 410-956-6637 or e-mail us at email@example.com. Please include the child's legal first and last name with date of birth. Please allow us 24 hours to complete your requests.
How to get my child's medical records copied?
Print out the Medical Release Form, complete and return to our office. Once we receive the signed release form we will contact the parent with the cost of copying. After payment is received, we will process your request and notify you when the records are ready for pcik up. There is a cost of $0.76 per page. If the record is being sent to someone other than parent/patient, there will be an additional fee of $22.88. Please allow 2-3 weeks for copying. For more information visit www.mbp.state.md.us.
Do you offer prenatal visits?
Yes! South River Pediatrics offers a comprehensive thirty minute prenatal visit for expecting parents at no charge. These visits are reserved for the last appointments of the day, Monday-Friday. They are designed to offer the opportunity for you to tour the office, have questions answered by our knowledgeable nursing staff, and meet one of our providers (either a physician or a nurse practitioner.) We provide you with valuable literature that will set you at ease during this exciting journey.

We prefer to schedule all visits in order to provide our patients with ample time in with the providers and to avoid extended waiting time in the lobby. If you are later than 15 minutes for a scheduled “sick” or “follow-up” visit, we will do our best to work you back into the schedule, or offer an appointment we have with another provider if there is availability. Please be aware, however, that we may be forced to ask you to reschedule out of courtesy to other scheduled patients that may arrive in a timely manner If you are later than 15 minutes for a well-child visit, we will need to have the appointment rescheduled.
If you are unable to keep your appointment, please call and reschedule, or notify the office of the cancellation. If you are running late, please notify the office but please review the late policy mentioned about to become familiar with our procedures. If it is a well-child visit that is cancelled or rescheduled on the same day, we do charge $40, unless we are able to fill the appointment.
For an appointment that is sick or follow-up visit that is cancelled the same day, there is a $25 fee unless we are able to fill the appointment.
After Hours/ Emergencies
If you feel your child is in the midst of a medical emergency, immediately dial 911. If you would like to speak to a nurse after hours to ask questions, you can call (443) 481-4000. After 1:00 a.m., you can reach a doctor on call, but please be aware there is a Charge for Non-Emergency returned calls. The number to reach a doctor on call is (443) 481-3076.
Please save non-urgent questions such as appointments, prescription refills, school forms, and referral requests for regular office hours. Please note that most insurance companies now offer an after-hours service which puts you in direct contact with a physician through their services, and most of the services provided through them are more likely to be covered by your insurance company. This information is often easily attainable on the back of the insurance card under which the child is covered.
